Requirements

The following requirements and reminders apply for the state of Virginia. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.

• You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
• You must fax the requestor's official state-issued photo ID. Virginia Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.
• A signature under the copied ID is also required. Unsigned applications will not be processed.
• Billing address for credit card used must be correct. Records will only be sent to billing address of credit card used.
