Requirements

The following requirements and reminders apply for the state of Texas. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.

• IF YOU WERE ADOPTED OR HAD A LEGAL NAME CHANGE (OTHER THAN MARRIAGE), YOU MUST CONTACT THE TEXAS STATE RECORDS OFFICE (AUSTIN) DIRECTLY TO ORDER A RECORD.
• IF YOU ARE REQUESTING THE CERTIFICATE TO OBTAIN A PASSPORT, YOU MUST CONTACT THE TEXAS STATE RECORDS OFFICE (AUSTIN) DIRECTLY TO ORDER A CERTIFICATE.
• Eligible requestors include self, parent, spouse, adult child, or demonstrate a legal right in order to obtain a copy of the record.
• You must fax the requestor's official state-issued photo ID. Texas Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.

You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
