Requirements

The following requirements and reminders apply for the state of Pennsylvania. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.

• You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
• The Pennsylvania records office requires The 3-digit CVC (last 3 digits on the back of the credit card if V/MC) or the 4 digit CVC on the front of American Express cards.
• Certificate will be only shipped to correct billing address of credit card used.
• Due to the billing address issue, any pre-paid credit card, gift credit card, or a credit card not issued in the United States will not be accepted by the Pennsylvania records office for an online order.

The following prices are charged by the state of Pennsylvania for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.
