Requirements

The following requirements and reminders apply for the state of Oregon. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.

• You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
• You must fax the requestor's official state-issued photo ID if you want Express Courier shipping. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.
  A copy of an I.D. is not required for certificates mailed via US Post Office.
• The Oregon records office requires the full names (including full middle names) of the persons listed on the certificate (certificate holder and both parents).
