Requirements

The following requirements and reminders apply for the state of New Jersey. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.

• You must be an immediate family member (spouse will require marriage certificate)or demonstrate a legal right to the record in order to obtain a copy of the record.
  A married female who has changed her last name due to marriage must provide a copy of a marriage license.
• You must fax the requestor's official state-issued photo ID. New Jersey Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.
• A signature under the copied ID is also required. Unsigned applications will not be processed.
• Photo ID address, billing address, and shipping address must all be the same.
• Due to the billing address issue, any pre-paid credit card, gift credit card, or a credit card not issued in the United States will not be accepted by the New Jersey records office for an online order.
• If you do not have a valid state-issued PHOTO ID with address, you must provide 2 alternate forms of ID with address. Acceptable alternate IDs with address are:
  • Non-photo state-issued Drivers License
  • Vehicle Registration
  • Insurance Card
  • Voter Registration
  • Passport
  • Green Card
  • County ID
  • School ID
  • 2 Utility Bills
