Requirements
The following requirements and reminders apply for the state of North Dakota. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- Eligible requestors include self, parent, spouse, adult child, or demonstrate a legal right in order to obtain a copy of the record.
- You must fax the requestor's official state-issued photo ID. North Dakota Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.

The following prices are charged by the state of North Dakota for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.

You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 1-888-803-1119.
