Illinois Records
The following requirements and product information are valid only for the state of Illinois. Read the requirements and then select an ordering method from the Product Information table.
Cook County births must order separately via the Cook County ordering section.
All other births in Illinois use the Illinois (all but Cook Co.) ordering section.
If the birth certificate is needed to obtain a passport (even if birth took place in Cook County), the application must be placed with the Illinois State records office

Requirements
The following requirements and reminders apply for the state of Illinois. Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- Eligible requestors include self, parent spouse, or demonstrate a legal right in order to obtain a copy of the record.
- You must fax the requestor's official state-issued photo ID. Illinois Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.
- A signature under the copied ID is also required. Unsigned applications will not be processed.

Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 1-888-803-1119.

The following prices are charged by the state of Illinois for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.

- Eligible requestors incude self, parent, spouse, or demonstrate a legal right in order to obtain a copy of the record.
- If the certificate holder has either been adopted or has had a legal name change (other than marriage), or the certificate is needed to obtain a passport, the application must be processed through the Illinois State records office (no regular mail delivery offered).
- You must fax the requestor's official state-issued photo ID. Illinois Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID. Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records.
