Requirements
If requesting an Authorized Certified Birth or Death Certificate, applicants must provide the following documents. This is a State of California ID requirement and must be faxed to us at 1-888-803-1119.
- Sworn Statement
- We apologize, but we do not currently offer this service for adoptions and legal name change.
If you were adopted or had legal name change (other than by marriage), your birth certificate can only be obtained through the State of California's Birth Records Office in Sacramento. - You must be an immediate family member or demonstrate a legal right to the record in order to obtain a copy of the record.
- Certificate of Acknowledgement Notarized in the presence of a Notary Public
- Requestors for Orange County births must also fax a copy of their Drivers License

County Information
Knowing your correct county is very important. If you place a certificate order through the wrong county, you will be charged a non-refundable search fee and and a "No-Find Letter" will be sent to you. Note that not all counties allow Internet orders for all certificates; you will need to contact the State Office for these orders.
The following prices are charged by the state of California for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.

You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
